READ INSTRUCTIONS BEFORE BEGINNING SEARCH

3 WAYS TO SEARCH FOR RECORDS

1. Use the "Facility Name" box and type at least the first three (3) letters of the Facility Name, e.g. McD, in the Name of Facility Box, and use the drop-down menu to locate the facility; or
2. In the "Address" box, type the address and street without  the suffix, e.g. 1234 Main, in the address box; or
3. Use the "Text Search" box for keyword searches.

VIEWING REPORTS

• To view a report, in the “VIEW” column, quickly click two times on the picture/icon of a sheet of paper.

• If there is a problem viewing the report with the E-Bridge viewer, select the row and then click PDF to display the report as a PDF (Adobe Acrobat Reader required).
