"DIET" Healthcare Provider Toolkit
The DIET (Dieticians Involved in Education & Training) program of the Cuyahoga County Board of Health, supported by Steps to a Healthier Cleveland, provides a FREE comprehensive toolkit with hundreds of ideas, resources, tools, and out-of-the-box programs for healthcare providers.  The toolkit focuses on assessment and treatment of childhood obesity but also includes resources related to asthma, diabetes, tobacco use, physical activity, nutrition and more.
